Case of the Week

A dainty small bowel gastrinoma in a patient with multiple endocrine neoplasia, type 1 (MEN1) resulted in an enormous lymph node metastasis. This phenomenon has probably resulted in at least some reports of primary gastrinoma of lymph node; tiny duodenal gastrinomas can be difficult to detect during surgical exploration and would likely be encompassed in a Whipple resection.
